STATE RECREATION SURVEY
We are conducting a short survey and would like your participation. This study is intended for anyone age 18 or older. By clicking the button below, you will be asked to complete a brief questionnaire. All of your answers will remain confidential. Your maximum total time commitment in the study is approximately 15 minutes.
At the conclusion of the survey you will have an opportunity to enter a drawing for one of many gift cards to your choice of retailer ranging from $50 - $500. Total prizes equals $3500. Only one entry per participant. Prize winners will be notified via email and phone.
